I am pleased to authorize a payroll deduction gift to be taken from my wages each pay period. I understand that these deductions will go to the PVM Foundation to provide financial support for the mission of Presbyterian Villages of Michigan. All funds will go to the area of greatest need, unless designated otherwise. As a 501(c)(3) organization, gifts to PVM Foundation are tax deductible to the fullest extent of the law. This payroll deduction will remain in effect unless the Foundation office is notified in writing of a change or cancellation. I will not need to re-sign a payroll deduction form each year.
